The format below is required when posting a ban request on someone. If you do not follow it then your request may be ignored until its fixed. Both the topic and body have its own format, and you can easily hit the "select all" button and copy and paste it to make it real easy.

Describe (in detail) what happened:

If you don't know one of the fields just put something along the lines of "Unknown".
